These services consist of integrated living support. While some individuals may require 24-hour supported living, others are better served with in-home Supports options tailored around their individual and family. Therefore, we tailor our residential-based habilitation services to each individual, so that they may successfully reside in their own homes, or with natural or adoptive families.
Each participant receives an Individual Service Plan which outlines goals and objectives that are designed to ensure the health, safety, and welfare of each participant. Our goal is to help individuals develop and retain the necessary skills to reside in their own homes.
Activities Include:
- Direct supervision, monitoring, and training to implement the Person-Centered Individualized Support Plan (PCISP) outcomes for the participant through
- Assistance with personal care, meals, shopping, errands, chores, leisure activities, and transportation (Note: This excludes transportation that is covered under the Medicaid State Plan).
- Coordination and facilitation of medical and non-medical services to meet healthcare needs. These could include physician consults, administering of medications, development and oversight of a health plan, utilization of available supports in a cost-effective manner, and maintenance of each participant’s health record.
- Assurance that direct service staff are aware and active individuals in the development and implementation of PCISP, Behavior Support Plans, and Risk Plans
