Structured Family Caregiving

Structured family caregiving (SFC) is a living arrangement where an individual resides in the private home of a primary caregiver. This primary caregiver may be a a foster care provider, or a family member other than the individual’s spouse, parent, or legal guardian.

SFC programs provide individuals with the means to work toward personal goals while receiving individualized support and care through a Person-Centered Individualized Support Plan (PCISP).

Activities Include:

  • Personal Care and Services
  • Chore Services
  • Attendant Care
  • Companion Care
  • Medication Oversight
  • Respite Services
